I am voluntarily participating in the Color Wars Run/Walk event at MHC Academy. I assume all risks and hazards, including personal injury, related to this event. I waive and release all rights and claims for damages against Mary Help of Christians Academy, all property owners, related companies, employees, agents, volunteers, sponsors, and other participants for any and all injuries or other damage related in any way to this event. I agree to hold all the entities and persons listed in this release harmless, and agree to indemnify and defend them. If I am signing on behalf of my child/children, I represent that I am the child’s legal guardian and understand that this document is binding against the minor. I understand that this is an important legal document and that I can consult with an attorney if I have any questions. I also guarantee that I am fully covered by the insurance company listed below. I also give permission for the Mary Help of Christians Academy to use photographs from this event of myself or my children.*
